You have several factors to keep in mind.  Your focus, whether AF or manual....it should be accurate.  Misfocusing at such thin DOF...it just ads to more issues.  Considering child movements, the shutter should be fairly fast.  As to DOF, you need to pick the spot where the camera will be at 90 deg to the child.  With thin DOF (especially close) you can get eyes in focus but lips and chin could be already OOF....depending on the angle of the camera.

I'd suggest you test the DOF on steady subject like bricks (at same level), fence, etc....and you'll be able to determine how much you truely have in focus.  Sometimes the lens could be doing front or back focus.  In any case, it should help.
