The plug on flash only has a guide number of 5 @ 100 ISO and using 1/200 s shutter speed this is only going to amplify his problems. The poster seems to think shutter speed will have no bearing, of cause it will.

The way you state it here, you are relying heavily on ambient light to prop up the flash. I am concerned with flash exposures only and the way they are metered with minimal, if any, ambient light support.

Use a fast shutter speed to eliminate the ambient and see how your TTL behaves then.
