The other thing to check is how well centered the screw is over the driving bit in the adapter. Generally there will be a bit of slop where you can wiggle the lens back and forth in its locked position. You want it centered between the two stops that will block it from rotating further or back. If it's all the way against one stop, the driver bit will tend to bind, which is what makes the focus less smooth.
