The most likely reason is the aperture ring on the lens was not a locked at f16. This ring can be unlocked for those who have a suitable camera and prefer to set the aperture manually.

It is possible the stop down mechanism on the lens is broken.

If you have only AF-s type lenses it is possible that the lever on the front of your camera body at 10 past the hour just outside the bayonet mount is broken. If this happens AF-s type lenses still work but older screwdriver autofocus lenses do not work properly. This problem is very rare. I have had it happen only once in 13 years using Nikon, after a fall holding a camera and lens.
