I encourage you to keep on doing your exercises. It took me about 6 months to return to fully pain-free happy use of my relatively light 400mm f/5.6. I keep on doing the exercises as prevention. If I think that I am going to be in one spot for any length of time, I deploy either my monopod or tripod, whichever is with me. I am back to my "run and gun" hand-held use for short spurts, though. I am a middle-aged woman, not particularly athletic, what strength I have is in my legs (hiking). I don't know that many birders who hand-hold their 10+ pound lenses.
