I'm not sure what that thing is supposed to do.  Looks like it would just set up another focal point for vibration.

If you carefully balance the lens on the head, you minimize torque and strain.  I mark the foot of my 400 2.8 and 200-400 with lines indicating the balance point on my cameras.  Makes setup real fast.

Use a decent ballhead and tripod.  Certainly check yours before chunking down on an expensive monster, but if you need a new one, get a good one.

Consider a Wimberly gimbal if you track active subjects.

The hand draped over the lens is a simple and very useful technique.  Practice.  The big lenses take a fine hand and careful prep work to get the best results.
