I don't know how Lightroom does it, but it has since version 1 and I'm using the current version 4.3

There will be a mapping done on all cameras at the factory.  When we do one, it adds any new ones to the list.

I've seen a new 5D2 that looked like they had forgotten to do it, maybe 20 or so hot pixels.  After the mapping procedure, magic - you could see them disappear as you describe (in Camera Raw here).
