What I do not understand is why this isn't being picked up in DxO's supposedly accurate Sensitivity (Ssat) measurements. Steen Bay has reasonably suggested that Ssat is not really 69. Iliah also seems to think so. If it walks like a duck, quacks like a duck...?

I always maintained that the characteristic curve near saturation in non-linear, and that sensor/camera manufacturers knowing that sometimes intentionally clamp at highlights. If highlight clamping occurs it is difficult to determine the full well capacity at "base" ISO and LO settings. Such shoulder, if it exists, also may be viewed as a sort of "varying ISO". If extrapolation of full well and/or noise does not work as expected it is a sign that something happens. It can be either in the sensor itself (say, drain of overflow is not stable enough, and the higher is the fill factor the more problematic is drain; or some sort of additional circuitry is affecting the readings), it can be in digital, and it can be a combo.

Interesting analysis of the possible causes.  And for it not to be reflected in DxO's Ssat reading, perhaps a little data pre-conditioning before storing the output of the ADC to the NEF?
