I've been using Blurb for a few years. They have three options, the one with the most control being PDF files made with InDesign, which I use. This way the only limiting factor in the design is one's own imagination. Their plug-in will create PDFs with 300 dpi resolution. If you use any images with less than this you'll get a warning but can still go ahead. Their two other design options are Bookify and Booksmart, neither of which I've used.
They will sell book for you online and you can specify your markup. What's reasonable depends on how professional your book is, and what you think people would be willing to pay. I take $10 on most of mine.
