The thing to keep in mind is that as soon as you brought up the levels, it is no longer an ISO 200 shot.  A one stop push in PP, for example, is *exactly* the same as having shot at ISO 400 in the first place.  And then, you are looking at an area that is in pretty deep shadow, meaning it will have the same amount of noise as a more properly exposed area would have in a shot at ISO 800 or higher.  But presumably, neither you nor the camera are actually applying the same sort of NR you'd have applied had you actually shot at ISO 800, meaning what you see is probably similar to what you might have seen had you shot at ISO 1600.

These are obviously just made up round numbers, but the general idea is - yes, if you shoot underexposed and push in PP, and then pixel peep the still-underexposed areas, and don't apply any NR, you are indeed going to see results consistent with a well-exposed and normally-processed image of much higher ISO.
