The important factors are the aperture and the distance between your camera and the subject, secondly the distance between subject and background. The sensor size matters for the circle of confusion. As your image shows, similar results can be achieved with almost any camera and lens.

Sometimes a big aperture leads to strange looking portraits. I. e.:

EOS 5D Mark III & EF 200mm f2.0 IS USM @ f2.0 and AWB

The aperture was too large (see full size). The whiplashes of the left eye are sharp, while the iris of the left eye is already out of focus. So is the right eye, as well as big parts of the beard.

Yes, I know you should not center faces.
