Need help with masking in/out hair

I've tried masking this hair in/out using different selection methods and refine mask with various combinations of options and creating an alpha channel and enhancing contrast (multiplying, manually painting in white/black, etc.) to make a more accurate mask.  Seems with both the best I've gotten has parts of the mask grey thus not completely masking out the original white background.  Making adjustments like shifting the edge in/out either leaves a white border around the selection or makes fine strands of hair partially disappear.  I'm guessing I'll have to simply remove some of the fine hair that is not coming out correct but what is the best way to handle the areas of fine hair that are producing the grey mask?
