Use a flash, move the camera for focus

I use a Raynox 250 with my Pentax 50-200.  I find the most productive M.O. is hand hold the camera with one hand and a wireless flash in the other.  Maneuver the camera until the subject is in focus (just set the focus ring to the farthest setting to provide the most working distance).  This is particularly important for moving subjects like bugs.

Oh, and F-stop should probably never be lower than about F/18.
