Your thinking is exactly online with what I'd consider. If the person is strongly backlit that would be enough to explain the effect. The other things you've considered are right down the list of what I was thinking of.

A lens element can be tilted and cause one side (or top/bottom or corner) to be softer than the others without a decentering effect in the center. But that doesn't sound like what you're seeing at all.
