looks to me like a front focus problem...

I can clearly see though in a few shots, where if they were shot at f1.4, that is a tiny area for the camera to chose focus, it needs a high contrast spot. When the focus point is hunting and there is no focal point at f1.4, it will sometimes choose the nearest point instead. You either have a front focus problem or you need to shoot differently than you are. Try single point, smallest focal point option. and if you have any handshake at all the camera can easily get confused in a bland focal area.
