Is OM-d autofocus accurate?

That's too bad. Unless you do a series of shots using a stable tripod with IBIS enabled, you just can't be sure it was a focus issue. I'm not saying it isn't, but methodical testing would help you narrow down the problem to a focus problem with the lens, a focus problem with the camera, or an IBIS issue.

A very easy test for focus would be to enable S-AF+MF. Then you can look through the viewfinder, let AF lock on the subject, and then use the magnify to check focus. Then see if turning the MF ring (still half pressing shutter) improves focus.

At any rate I think your experience was unusual. Whether the lens or the camera was at fault, I think exchanging the whole kit probably would have resolved the issue.
