Sounds like a problem of communication between body and lens regarding aperture. Was the aperture ring locked in the A position?

I've also had this sort of problem if the camera was set to bracket mode while using flash. Having forgotten it was set to bracket, I'd let up on the shutter release quickly enough that only one shot was taken. There was enough time for all my flashes to recycle between shots. One shot would be correct, the next overexposed, the next underexposed.

Those are the first two things I would check.
