I get diffusion with a hot shoe mounted flash by bouncing the light off a wall or ceiling. Outdoors I use a flash bracket to elevate the flash and use a LumiQuest diffuser. They are compact, lightweight so they do not stress the hot shoe, inexpensive, and they work great with all the light going forward toward your subjects. Nothing dumber than the Fong type of flash modifiers that waste 75% of the output from the flash.

Even using a piece of white cardboard attached to the strobe with a rubber band can be used to throw some of the light forward and illuminate your subjects' eye sockets and provide adequate fill lighting.
