You should definitely be honest, but you should also respect that others have different preferences and style than you. Also, a lot of portraits are taken of people that the photographer care about. If they are happy with the portrait it's probably because it is a good reflection of the subject's personality and je ne sais quoi.

I think very few are as deluded as to think that they are making high art and that others would pay good money for their shots, but that doesn't mean you can't share the enthusiasm for photography and for a shot they think really nailed it.

If people are asking for tips on how to improve, feel free to give constructive comments about framing and technique, and if you can't, just hold your peace.
