For hiking, backpacking, snowshoeing, snow climbing, and trekking, I use a padded Tamrac shoulder holster with the main pack's waistbelt through the back of the hoster. I have several sizes of the Tamrac holsters (varies by lens length) and like the fact that there is a single clip to the cover (in addition to zippers). With this system, the camera is solid (no swinging) and generally protected but is also readily accessible. Extra lenses hang in cases off either the waist belt or the main pack's shoulder straps. Works with anything from a small fanny pack to an 85 liter Lowe backpack.
