Put manual and card into box.
Put the other box insert into the box over the top of the lens pouch.

Sounds about how I remember it. One thing for certain, this lens was packaged to survive an earquake, truck roll-over and a bomb blast without any shock or impact damage. It seemed even more secure than the 300mm f/2.8 that comes in it's own miniature suitcase.
