Original Packaging of 70-200mm f/2.8 IS II

I received this lens several months ago and as you can imagine I was in a hurry to get out shooting. Everything was unpacked quickly with no idea how everything was packed by the factory. Now I would like to repack the lens as per the factory packing and for the life of me I just can't remember or figure out the original arrangement.
This is everything I saved.
1. Main Box
2. (2) Large square Styrofoam inserts.
3. (2) Styrofoam rings.
4. Large plastic bag, on left side of picture and folded in half.
5. Medium plastic bag, under large plastic bag.
6. Small plastic bag, on right side of picture under lens case.
7. Small piece of fabric material, on top of small plastic bag.
8. Lens
9. Tripod ring.
10. Lens hood.
11. Lens case.
12. Front and rear caps.
13. Owners manual and warranty card. (not shown)
Was the lens packed inside the lens case or was the case folded and separate?
What were the Styrofoam rings used for.

Did the lens ship with both caps attached, tripod ring attached or separate, lens hood attached or separate?

Can anyone help me repack this lens as per factory packing?
