Toddler Portrait Feedback

Hi, I did not know where to post this sample however I figure the lighting forum is where the best portrait pros are located.

I ran into a problem this weekend and do not know why or what may have caused my issue. I was shooting toddlers and they move quite a bit so I went ahead and set my camera to AI focus AF mode using just the center point (which was just on the inside of the left eye). But for some reason the photo was not in focus in the center but more towards the upper right.

Here is the sample of the photo:

Camera used was a Canon XSi
Sigma 24-70mm lens

Centerweighted average metering
Exposure Compensation
ISO Speed
Focal Length
45.0 mm

Any ideas or suggestions would be greatly appreciated!
