Good tripod head for Nikkor 70-200 VR?

I have the 70-200mm VR lens and have been using it on my Manfrotto 055PRO tripod and my Manfrotto 486RC2 ball and socket head.

My camera and this lens are quite heavy, heavier still if I use a teleconvertor, I think the tripod is just about strong enough to hold the weight but the head struggles so I think I need a stronger head. Are there any you could advise please?

It even struggles to keep completely still in every position when I'm using the 105mm macro lens.

Also if I'm getting a new head will it come with a better attachment to attach it to the lens (or camera) because the one I currently have only screws into one hole, that’s fine when attaching my camera to the tripod but the lens collar has two screw holes and sometimes it doesn't hold firm on the collar?
