How about an attached remote?

Another option-

Some cameras have accessory IR remotes, such as this one next to a Canon G6:

One of these remotes could be modified to relocate the IR emitter onto a short wire (3-4 inches, 10 minute job with a soldering iron), which could then be placed over the camera's IR receiver.

The small remote could then be affixed to the camera (anywhere you want) with some double-sided foam tape. The remote is simplified by nature, putting all the buttons on the same plane and limiting them to the "essentials". Having them all together would be perfect for single-finger (thumb?) operation, without having to adjust your grip on the camera.
