Here, the deck-hand is pulling the fighting fish alongside the boat, much like Hemingway's character did. The desparation of the moment, as the fish fights for his life, and that of the handler, as he fights to hold on illustrates the power, glory, and unforgiving nature of a life reliant on the ocean.
