From what I remember, a pixel can be in dead in any of several ways, depending on how the component photosites failed. It might be reading out as black, white, red, green, blue, or a mixed color. If the camera runs a remapping function, how does it recognize pixels that are dead in all of these possible ways? Is it necessary to run the remapping operation with the lens covered?

Obviously different camera system in my case, but my "stuck" pixel showed as white. I don't think it's necessary to have the lens covered to do a remap, as there's going to be a certain amount of light leakage even with a lens cap on.
