Beyond "hot pixels"

I started this post then did some googling about hot pixels and have modified my query. I was able to confirm that Lightroom automatically fixes the big hot pixels but I have a tremendous amount of smaller specks throughout my long exposure images. They show up either white or red. Is this sensor noise/heat? Dust on the lens/sensor showing up? Any way to clean it up without having to clone each speck out manually. Obviously when viewed full size they don't show up, still annoying.

This one is much exaggerated due to the editing done but still, it's crazy

this was 20s at ISO200 with 8mm fisheye, shot raw
