I am a dental surgeon. We have a Canon 40D with huge macro lens and ring flash. The ring flash is necessary to properly expose anything inside the mouth, for example pictures of individual restorations or surgical procedures (implants, grafts, pathology removal, etc.). The cheek and lips interfere with ambient or overhead light so the ring flash is a must. You need large diameter lenses to secure a big ring flash so naturally APS Canikons fit the bill. It's a huge monstrosity but it works and we just keep the setup in the office at all times.

Smaller cams are perfectly fine to capture a smile.
