Documentation for scientific photography and video - best practices

We need to build a documentation sheet for photos and videos taken in labratory, outdoor, and and impromptu scientific experiment settings. Does anyone know of the "best practices" for doing so? I have found some relevant templates in peer-reviewed papers, but would like some ideas from the knowledge I often see on DPreview.

We use cameras ranging from a G1X for stills to Phantom high speeds for video, and everything in between. Envisioning separate sheets for stills and video. Any input or available templates are much appreciated.
