Sling straps - do you trust them?

I've had a Carry Speed sling strap for just over a year.  It's not had a lot of use but on one early occasion when I checked the attachment I noticed it was loose so have been careful ever since to (finger) tighten it firmly.

So I was mortified to hear the clatter of my month-old camera falling from it yesterday!     Fortunately the cap and hood were in place and apart from the most minor scuff on the edge of the hood it appears to be fine.

I don't have the tripod plate attached, just the screw fitting.  So how tight should these be done up?  Should I use a tool to force it tight so it can't be undone by hand?

My first task is to fabricate a little safety cord between this strap and the strap lug on the camera before I ever attempt to use it again!
