I don't know how widespread the problem is, but it does happen. I have a DA55-300 that is noticeably decentered over much of its zoom travel. Fine if you're only interested in what's in the middle of the shot, but hopeless for landscape or architecture. Very frustrating at times. (In fairness, the first two Sigma 17-70s I had were shockingly worse!)

I've had a couple of decentered second-hand primes, but that might have been the result of dodgy repairs or servicing in the past. Or physical impact. All the new Pentax primes I have bought have been fine.

Zoom lenses are much more complex in construction, and more liable to mechanical faults, which is why I tend to avoid them these days. I can only afford the cheap ones anyway.
