I suppose a follow-up question is: How big is the physical size of the primary sensor? A full-frame camera is 36mm x 24mm.

The largest sensor--the wide field camera in the advanced camera for surveys--is about 62mm x 62mm.

Ok. However, I thought I'd seen images of the moon from the HST. Would a portion of the moon that was in shade be imaged? Would the moon be moving too fast in relation to the HST?

Not sure if the Moon's relative speed would be an issue.

I suppose the 4k x 4k sensor would be a relatively modern add-on from a servicing mission?

Yes.

There are two 4K x 4K sensors on the HST. The first (the ACS WFC) was installed in 2002 while the other (in the WFC3) was installed in 2009.
