It would help if you could post a photo with exif. Also, close in objects with it wide open your dof is going to be very shallow, can you be sure you are not slightly moving forward or backward?

When viewed at 100%, the first of the two images above seems to be focused not on the hands or the object being held, but on the black object, that appears to be a leather armrest of a couch or chair or something (I can't tell what the object is.)

I can't tell you why you're getting a green box around your intended target; since it's a new camera, perhaps the settings are not set the way you think they might be. When stuff like this happens to me, I have to search through the menu's looking for settings I might need to change, and referring to the manual to verify how the mechanisms operate and are controlled.
