How do you avoid flash in the eyes or the "catch light"?

I have looked at many studio produced portraits and other professional flash photography pictures.  I have taken quite a few myself.  Almost all of the photos I have seen I can see those lights in the eye.  Even in photos of the President, I can still see what looks like a flash/lighting stand in his eyes and the light across his face screams "I used a flash".

I know about umbrellas, soft boxes, etc. all too well.  These items are helpful, but you can still see that flash in the eyes or that flash of light across the forehead.

I know this is not an easy topic to answer.  I am guessing the answer is the position of the lights and the subject.  Software might also play a part in it.

So what is your method to avoid the "catch light" or other tell tale signs of flash usage?
