I experienced the double image "shutter shock" last weekend shooting with a cheap 135mm adapted lens.  I was holding the camera by the lens and only activating the shutter with my other hand.  This was the first and only time I've seen it.

The camera was an E-PL5, and the lens (which also had severe purple fringing) was disassembled to use for parts to fix another lens.
