Yes. We tightened it up, but it self-loosened. There's a collar that pulls the shoe foot into the flash, rather than the more common tightening nut that clamps the shoe to the foot. The work around was to use a cold-shoe with a screw clamp. IIRC, I talked her into (read: gave her my) Stroboframe shoe until she could send them in for repair.

I thought it was a novel design, but something must have gotten stripped inside the flash.

Again, I apologize for assuming this was the T5dR. I'm not that familiar with these flash units. Thanks for pointing out this must have been the Trio. Between the foot issue, and the bulb seemingly spontainiously ejecting, I have to say that I'm not impressed with the build quality, but I was impressed by the volume and quality of light produced.
