No, the picture is recorded exactly the same way as usual.  It is no more rotated than when you shoot portrait vs. landscape.

The only difference in the processing of the image when you shoot upside down is one byte in ExIF info.  The gravity sensor knows where the “top” is w.r.t. the recorded image, and this is marked in ExIF.

If you use a smart enough image viewer, then THIS VIEWER will turn the image upside down (basing on the info).  But if you strip ExIF, the image will be shown “as shot”.
