I think you should isolate whether it is subject movement, camera shake, or camera/lens focus problem. You said that other area instead of eye is sharp, that probably rule out subject movement and camera shake. But to be sure, mount camera on tripod, use a toy and set your camera to use center focus point. If focus is still off, your lens may have a back/front focus problem.
