Not a dumb question at all.

See this thread I started in May 2011 in which I asked why my shots were underexposing when using a Canon 580 EXII under the following conditions

a) with a modifier on the flash (flash is on camera or off with OC-E3 cable - makes no difference)

b) the flash head horizontal and pointing straight forward

c) using an EF lens (eg Canon 24-105mm zoom) where distance information is being sent to the flash

Bottom line is learn to control the light manually and use E-TTL only when you have no choice.
