You may be right: it seems that LR caches the sizes actually previewed so it depends on the usage history of the files. I've checked some .lrprev files on my machine (.lrprev is a pyramid format, aka mipmap aka multi-resolution - I wrote a codec for these) and some are thumbnail sized, other much larger (i.e. decent previews as you say, e.g. 1440px) and some are definitely full-resolution.

I no longer have LR4 itself installed so I cannot easily check what triggers the full-size preview caching, perhaps you need to zoom to 1:1 once.
