Password Manager

Passwords used at my PC are only a minor headache. Now that I finally have a smartphone I'm beginning to think it's a problem. Do you use an app to hold your passwords? Do you think a strictly"on-device" approach is OK or have you found cloud connections (backup and sync with PC and other services) is useful?
