In Japan this is especially difficult because the culturally accepted indoor winter temperature is over 20 C.  Going from -25 to +25 means I quickly drop my gear in a unheated room (5 or 6 C) and let it adjust there for a couple hours.  Don't work the zooms, don't dismount lenses (sensor will condense), don't use a blower to remove debris.   When I am out driving + shooting I tend to leave the heater off completely, or just the blower on to keep the windows clear.
