The Noise Reduction function in Lightroom/Adobe Camera Raw is usually sufficient for my needs but on those occasions when I need to use a third-party noise reduction tool, I skip noise reduction in the raw file.  Instead I convert the raw file to a 16-bit TIFF and then process it in Photoshop using Topaz Denoise for noise reduction.

Noise reduction ideally should be done early in processing; global color and tonal corrections should be done first, followed by noise reduction.  The reason is that tonal/brightness corrections will reveal the true extent of noise in the image.  Also, if NR isn't done early its possible that your later localized corrections could amplify noise .e.g. noise influencing your masks, etc.
