Many serious BIF and wildlife photographers who use super telephoto lenses use gimbal mounts such as those made by Wimberley or Jobu.  They tend to be expensive but are worth it.  They allow you to pan smoothly as well as tilt to photograph the birds as they fly overhead.  Before I bought my Wimberely, I tried many other solutions but none compared.  Wimberley offers a free trial period (or at least they did last time I looked) where you can use either of their designs and return it if you don't like it.
