The Wimberley II gimbal is among the "best" tripod heads, but not the least expensive. I'm also having good luck with a gitzo monopod and the manfrotto 234 mini-head. I think that would work really well with a 400mm.
I tore a ligament in the elbow holding heavy lenses. Now I'm using a Velbon CF monopod and the Manfrotto 234 monopod head. It's excellent and I carry the combo with a BlackRapid strap so I don't have to lift anything.
