And if I'm doing a run nd gun documentary style shoot, I'm expected to know how every shot is going to go and have it all preset before hand? I'm sorry but that's just stupid, I need to be able to react to what's happening infront of my, and I don't trust my camera to make the decision of how to react (and never will, no matter how fast or otherwise with autofocus is).

As for you last example focus peaking would help. As something comes into the shot you simply turn the ring gently and watch the plane of focus move forwards (or backward) until it is on the intended subject.
