My 70-200 does have a tripod foot  and of course I have an Arca-Swiss compatible  plate for it as well for use with my BH-55 ball head.

Like you say, for non collared lenses I would need a rail, but I have those too which I use for macro work.

Thus the gimball head can certainly be easily made to work in all cases with all lens types.  The question is not really how to do this, but is there any reason I should want to.
