I've been in contact with Ricoh/Pentax technical support. They asked me to send some example photos with the error (with preserved exif data), which I did. I have not yet heard anything back, but I'll give them another couple of weeks...
Edit: There are no info in the EXIF of flash intensity or focus distance, from what I can dig out.
I'd like to hear what Ricoh have to say....
