Threshold adjustment layer / clipping mask not working correctly in CS6

seems the preview is the culprit.

After several more tests, I have figured out that the threshold adjustment layer preview is simply inaccurate. More accurate results come from applying the threshold direct to the layer instead of using an adjustment. Unfortunately, the preview in the filter is also inaccurate, and wildly so.

A bigger issue is that trying to accomplish what I want (the top image in the linked image I posted), I have to alter my algorithm.

The following is closer to what I need and works on a high rez (21MP) image.

Make layer
Fill
Using: foreground color
Add Noise
Distribution: gaussian
Percent: 150%
With Monochromatic
5568165
Gaussian Blur
Radius: 5 pixels
Levels
Preset Kind: Custom
Adjustment: levels adjustment list

Channel: composite channel
Input: 103, 112
